A Case of Placental Site Trophoblastic Tumor

Abstract

|
Placental site trophoblastic tumor (PSTT) is a rare distinct neoplastic proliferation of intermediate trophoblasts. Most of PSTT have benign clinical courses, but some fatal outcome is also reported. We report a case of PSTT in a 26-year-old woman who had vaginal bleeding and transient heavy proteinuria. Grossly, the uterus was diffusely enlarged without distinct mass formation. Microscopically, cords or clusters of intermediate trophoblasts were diffusely infiltrated in the myometrium. In immunoperoxidase staining, the trophoblasts revealed a strong positive reaction for human placental lactogen (hPL). Human chorionic gonadotropin (hCG) was focally positive. This case is believed to be unique in two aspects. There was no recognizable mass grossly although multiple endometrial curettage and biopsies were perviously done. Another is a possibility of PSTT with nephrotic syndrome as in the previously reported cases. We also discussed the relationship between PSTT and glomerular lesions.
